We are happy to file necessary forms to your primary dental benefits insurance company. To file the claim on your behalf, we must be provided with the correct insurance information and a copy of your insurance card. We will call your carrier to verify coverage and benefits for your dental plan. The insurance company will provide us with a description of benefits which is not a guarantee of payment.
